Latar Belakang: Medication error adalah suatu kejadian yang tidak hanya dapat merugikan pasien tetapi juga dapat membahayakan keselamatan pasien yang dilakukan oleh petugas kesehatan khususnya dalam hal pelayanan pengobatan pasien. Medication error dapat terjadi di dalam tiap proses pengobatan, salah satunya pada fase prescribing. Tujuan penelitian ini adalah untuk mengetahui angka kejadian medication error pada fase prescribing dalam pelayanan pengobatan di RSD Mayjend HM Ryacudu Kotabumi. Metode: Penelitian ini merupakan penelitian deskriptif cross sectional. Subjek penelitian ini adalah resep yang ada di instalasi Farmasi RSD Mayjend HM Ryacudu Kotabumi. Penelitian ini menggunakan metode total samplingyang mengambil resep di bulan Juni- Oktober 2016 di instalasi Farmasi RSD Mayjend HM Ryacudu Kotabumi. Hasil Penelitian: Angka kejadian medication error pada fase prescribing menunjukan 63,6%.Dimana dokter spesialis melakukan medication error sebesar 72,5% dan 43,4% yang dilakukan oleh dokter umum. Kesalahan fase prescribing pada bagian inscriptio terhadap pasien rawat jalan RSD Mayjend HM Ryacudu Kotabumi sebesar 58,5%. Angka kejadian kesalahan pada bagian prescriptio sebesar 63,6%, signatura sebesar 25,4%, dan pro sebesar 81,9%. Sedangkan angka kejadian pada bagian invocatio dan subscriptio sebesar 0% Kesimpulan Penelitian: Angka kejadian medication error sebesar 63,6%. Terdapat hubungan antara tingkat pendidikan terhadap medication error pada fase prescribing. Kata kunci: Angka Kejadian, medication error, fase prescribing. ABSTRACT Background: Medication error is an event that can not only harm the patient but also may endanger patient safety carried out by health workers, especially in terms of patient treatment services. Medication errors can occur in each of the treatment process, one of them on prescribing phase. The purpose of this study was to determine the incidence of medication errors in prescribing phase on medical services at Mayjend HM Ryacudu District HospitalKotabumi. Methods: This was a cross sectional descriptive study. The subjects using prescription in the installation of Hospital Pharmacy Mayjend HM Ryacudu Kotabumi. The total sampling method is applied by using prescription on June-October 2016 in the installation of Hospital Pharmacy Mayjend HM Ryacudu Kotabumi. Results: The incidence of medication errors in prescribing phase showed 63,6%. 72,5% of specialist doctors did medication error and only 43,4% general practice doctors did medication error. Error on the inscriptiopart prescription to outpatient hospitals HM Mayjend Ryacudu Kotabumiamounted 58.5%. The incidence of errors on the prescriptio part amounted to 63.6%, 25.4% Signatura, and pro amounted to 81.9%. Whereas the incidence in the invocatio and subscriptio at 0%. Research Conclusions: The incidence of medication errors amounted 63,6%. There is a relation between the level education with medication error in prescribing phase.
