Abstrak

Latar Belakang: Sepsis neonatorum merupakan sindrom klinis penyakit sistemik disertai dengan bakteremia pada bayi dalam satu bulan pertama kehidupan. Menurut data rekam medis di RSUD Dr. H. Abdul Moeloek Provinsi Lampung, pada tahun 2017 – 2019 didapatkan peningkatan kejadian sepsis neonatorum, dimana tahun 2017 terdapat 193 kasus, tahun 2018 terdapat 242 kasus dan tahun 2019 terdapat 317 kasus. Penatalaksanaan sepsis neonatorum diperlukan pola gambaran mikroorganisme sesuai dengan karakteristik serta patogen terbanyak yang nantinya, diuji sensitivitas antibiotik yang dapat digunakan sebagai terapi empiris, serta faktor risiko yang berhubungan dengan sepsis neonatorum. Tujuan penelitian ini adalah untuk mengetahui prevalensi, pola gambaran mikroorganisme, sensitivitas antiobiotik dan faktor risiko pada kejadian sepsis neonatorum di Unit Perinatologi RSUD DR. H. Abdul Moeloek Provinsi Lampung. Metode: Penelitian ini menggunakan metode kuantitatif dengan pendekatan cross sectional, mulai dari April-September 2022. Instrumen yang digunakan berupa data rekam medis, kultur darah dan uji sensitivitas antibiotik. Analisis data dilakukan dengan menggunakan perangkat lunak statistika SPSS dan Microsoft Office Excel. Sampel penelitian sebanyak 113 responden yang ada di Unit Perinatologi dan NICU RSUD Dr. H. Abdul Moeloek Provinsi Lampung diambil menggunakan metode total sampling. Hasil: Berdasarkan hasil analisis, didapatkan prevalensi kejadian sepsis neonatorum sebesar 40,7%. Mikroorganisme patogen penyebab sepsis neonatorum terbanyak pada bakteri gram negatif sebesar 44 sampel (95,7%), didominasi oleh Klebsiella pneumoniae dan Pandoraea sp sebesar 8 sampel (17,4%). Selain itu, didapatkan bahwa Meropenem (82,6%) merupakan antibiotik yang paling sensitif, diikuti oleh Ciprofloxacin (60,9%) dan Sulfamethoxazole (60,9%). Sedangkan, antibiotik yang paling resisten adalah Gentamisin (76,1%), dan Ampicillin- Sulbactam (76,1%). Variabel yang berhubungan dengan kejadian sepsis neonatorum di RSUD Dr. H. Abdul Moeloek, yaitu jenis kelamin (p value 0,037), usia neonatus (p value 0,003), usia gestasi (p value 0,011), dan berat badan lahir (p value 0,033). Simpulan: Prevalensi kasus sepsis neonatorum di RSUD Dr. H. Abdul Moeloek Periode April-September 2022 didapatkan 40,7%, dengan penyebab terbanyak adalah Klebsiella pneumoniae dan Pandoraea sp. Antibiotik paling sensitif adalah Meropenem, Ciprofloxacin, dan Sulfamethoxazole, sedangkan yang paling resisten adalah Gentamisin dan Ampicillin-Sulbactam. Jenis kelamin, usia neonatus, usia gestasi, dan berat badan lahir berhubungan dengan kejadian sepsis neonatorum di Unit Perinatologi RSUD Dr. H. Abdul Moeloek Provinsi Lampung. Background: Neonatal sepsis is a clinical syndrome of systemic disease accompanied by bacteremia in infants in the first month of life. According to medical record data at RSUD Dr. H. Abdul Moeloek, in 2017 - 2019 there was an increase in the incidence of neonatal sepsis, where in 2017 there were 193 cases, 242 cases in 2018, and 317 cases in 2019. The management of neonatal sepsis requires a pattern of microorganisms according to the characteristics and the most common pathogens which will later be tested for antibiotic sensitivity which can be used as empiric therapy, and risk factors related to in neonatal sepsis. The purpose of this study was to determine prevalence, the pattern of microorganism appearance, antibiotic sensitivity and risk factors in infants with neonatal sepsis in the Perinatology Unit of RSUD DR. H. Abdul Moeloek Lampung Province. Methods: This study was a quantitative with a cross sectional approach, between April to September 2022. The instruments used are the medical records, blood cultures and antibiotic sensitivity test results. Data analysis was performed using SPSS and Microsoft Office Excel. The research sample consisted of 113 respondents in the Perinatology Unit and NICU of RSUD Dr. H. Abdul Moeloek who were taken using the total sampling method. Results: Based on the results of the analysis, the prevalence of neonatal sepsis was found to be 40.7%. The most pathogenic microorganisms that cause neonatal sepsis were gram-negative bacteria with 44 samples (95.7%), dominated by Klebsiella pneumoniae and Pandoraea sp with 8 samples (17.4%). In addition, it was found that Meropenem (82.6%) was the most sensitive antibiotic, followed by Ciprofloxacin (60.9%) and Sulfamethoxazole (60.9%). Meanwhile, the most resistant antibiotics were Gentamicin (76.1%) and Ampicillin-Sulbactam (76.1%). The result of bivariate analysis showed all variables had a correlation with neonatal sepsis, such as gender (p-value 0,037), neonatal age (p-value 0,003), gestational age (p-value 0,011) and birth weight (p value 0,033). Conclusion: Prevalence of neonatal sepsis cases at RSUD Dr. H. Abdul Moeloek in April-September 2022 were 40.7%, with the most common causes being Klebsiella pneumoniae and Pandoraea sp. The most sensitive antibiotics are Meropenem, Ciprofloxacin, and Sulfamethoxazole, while the most resistant are Gentamicin and Ampicillin-Sulbactam. Gender, neonatal age, gestational age, and birth weight are associated with neonatal sepsis in Perinatology Unit and NICU of RSUD Dr. H. Abdul Moeloek.
