Konversi sputum merupakan salah satu indikator dari keberhasilan pengobatan TB. Pengobatan yang kuat akan memberikan hasil yang negatif pada pemeriksaan sputum penderita TB setelah masa pengobatan intensif atau 2 bulan pengobatan awal. Penderita tuberkulosis yang mengalami tertundanya konversi setelah masa intensif akan membawa risiko kegagalan pengobatan, resisten terhadap obat tuberkulosis. Tujuan dari penelitian ini adalah untuk mengetahui penyebab tertundanya konversi sputum setelah fase intensif pada pasien TB baru BTA positif. Penelitian ini merupakan jenis penelitian kualitatif dengan pendekatan Case Study dengan teknik samping total sampling. Jumlah informan pada penelitian ini sebesar 31 informan, penelitian dilakukan pada bulan Oktober 2022- Januari 2023. Penelitian ini dilakukan di 9 Puskesmas di Kota Bandar Lampung. Data dikumpulkan menggunakan Register TB 03 yang ada dipuskesmas selanjutnya dilakukan wawancara mandalam. Analisis data pada penelitian ini menggunakan analisis tematik. Hasil penelitian ini ditemukan 4 tema penyebab tertundanya konversi sputum yang tertunda yaitu kepatuhan minum obat, kebiasaan merokok, kebiasaan konsumsi alkohol dan adanya penyakit penyerta. Hasil kesimpulan yang diperoleh, perilaku gaya hidup seperti kepatuhan minum obat, kebiasaan merokok, kebiasaan konsumsi alkohol dan faktor lain seperti adanya penyakit penyerta menjadi penyebab tertundanya konversi sputum setelah fase intensif pada penelitian ini. Saran bagi pihak puskesmas diharapkan dapat melakukan monitoring dam evaluasi kinerja program TB. Kata Kunci : Konversi Sputum, Mycobacterium Tuberculosis, TB Paru. Sputum conversion is one indicator of the success of TB treatment. Strong treatment will give negative results on sputum examination of TB patients after a period of intensive treatment or 2 months of initial treatment. Tuberculosis patients who experience delayed conversion after an intensive period will carry the risk of treatment failure, resistance to tuberculosis drugs. The purpose of this study was to determine the causes of delayed sputum conversion after the intensive phase in new smear-positive TB patients. This research is a type of qualitative research approach case Study side technique total sampling. The number of informants in this study was 31 informants. The research was conducted in October 2022-January 2023. This research was conducted at 9 Community Health Centers in Bandar Lampung City. Data was collected using the TB 03 register at the health center, then an in-depth interview was carried out. Data analysis in this study used thematic analysis. The results of this study found 4 themes causing delayed sputum conversion, namely medication adherence, smoking habits, alcohol consumption habits and the presence of comorbidities. The conclusions obtained, lifestyle behaviors such as medication adherence, smoking habits, alcohol consumption habits and other factors such as the presence of comorbidities are the causes of delayed sputum conversion after the intensive phase in this study. Suggestions for the puskesmas are expected to be able to monitor and evaluate the performance of the TB program. Keywords: Sputum Conversion,Mycobacterium Tuberculosis, TB.
