PT. Gizindo Pangan Sejati is a company that produces wafer stick products with the brand "giziku". The purpose of this study was to determine the value of DPMO, sigma level and determine the dominant defects and factors causing defects in wafer stick products using six sigma method. The method in this study is descriptive, with the collection of production data and product defect data for one month. Data analysis refers to the principles contained in the Six sigma method, namely DMAIC (Define, Measure, Analyze, Improve and Control). The results of the analysis define there is a type of defect that causes the product to reject the wafer stick, the wafer stick is not rolled, the wafer stick is empty, and the wafer stick is broken. The measure analysis obtained the results of the average value of DPMO and sigma levels in February 2023 of 10716 and 3.8145 while in March 2023 the average value of DPMO and sigma levels was 12110 and 3.7936. Analysis analyze based on pareto diagram the dominant defect type in February and March 2023 is the unrolled wafer stick defect type which is as much as 2397kg and 1682kg. Based on the analysis of cause-and-effect diagram, the factors that cause product defects are factors that are old machines that cause unstable roll machine rotation, work sops that are poorly understood by employees, too thick cream dough ingredients that cause the dough to clump, employees are exhausted and the work environment is less comfortable because of the high production room temperature. The improvement recommendations provided include routine machine maintenance, providing job training to employees, providing clear SOP or work instructions and creating a comfortable working environment for employees. Keywords: Defect, Products, Six Sigma, Pareto Diagram, Repair Recommendations. PT. Gizindo Pangan Sejati merupakan perusahaan yang memproduksi wafer stick dengan merk “giziku”. Tujuan penelitian ini adalah untuk mengetahui nilai DPMO, sigma level dan mengetahui cacat dominan serta faktor-faktor penyebab cacat pada produk wafer stick dengan menggunakan metode six sigma. Metode pada penelitian ini yaitu deskriptif, dengan pengumpulan data produksi dan data cacat produk selama satu bulan. Analisis data mengacu pada prinsip-prinsip yang terdapat dalam metode six sigma yaitu DMAIC (Define, Measure, Analyze, Improve dan Control). Hasil analisis define terdapat jenis cacat yang menyebabkan produk wafer stick mengalami reject, yaitu wafer stick tidak tergulung, wafer stick kosong, dan wafer stick patah. Analisis measure diperoleh hasil nilai rata-rata DPMO dan sigma level bulan Februari 2023 sebesar 10716 dan 3,8145 sedangkan bulan Maret 2023 nilai rata-rata DPMO dan sigma level sebesar 12110 dan 3,7936. Analisis analyze berdasarkan diagram pareto jenis cacat yang dominan bulan Februari dan Maret 2023 adalah jenis cacat wafer stick tidak tergulung yaitu sebanyak 2397kg dan 1682kg. Berdasarkan analisis diagram sebab-akibat, faktor- faktor penyebab terjadinya cacat produk ialah faktor mesin yang sudah tua yang menyebabkan putaran mesin roll tidak stabil, SOP kerja yang kurang dipahamioleh karyawan, bahan adonan krim terlalu kental yang menyebabkan adonan menggumpal, karyawan yang kelelahan dan lingkungan kerja kurang nyaman karena suhu ruang produksi yang tinggi. Rekomendasi perbaikan yang diberikan antara lain, melakukan perawatan mesin rutin, memberikan pelatihan kerja kepada karyawan, memberikan SOP atau instruksi kerja yang jelas serta menciptakan lingkungan kerja yang nyaman bagi karyawan.
