Latar Belakang: Gagal jantung adalah salah satu penyakit yang sering menjangkit usia lanjut dan berkaitan dengan peradangan kronis. Rasio neutrofil limfosit (RNL) adalah salah satu penanda inflamasi yang mudah dan cepat pada berbagai kondisi, khususnya penyakit kardiovaskular. Tujuan penelitian ini adalah untuk mengetahui korelasi rasio neutrofil limfosit (RNL) dengan fraksi ejeksi ventrikel kiri pada pasien gagal jantung. Metode: Penelitian ini merupakan penelitian observasional analitik dengan desain cross-sectional. Data diambil dengan teknik purposive sampling. Data yang digunakan adalah data sekunder sejumlah 88 rekam medis yang memenuhi kriteria inklusi dan eksklusi. Penelitian ini menggunakan uji analisis Spearman Rank. Data rekam medis diambil di instalasi rekam medis RSUD Dr. H. Abdul Moeloek Provinsi Lampung sepanjang tahun 2023. Hasil: Hasil statistik penelitian ini adalah didapatkan sig. (2-tailed) sebesar 0,006 (<0,05) antara rasio neutrofil limfosit (RNL) dengan fraksi ejeksi ventrikel kiri dengan koefisien korelasi -0,290. Berdasarkan analisis ROC, didapatkan cut-off point RNL terbaik sebagai prediktor fraksi ejeksi ventrikel kiri adalah 2,67 dengan sensitivitas 70,7% dan spesifisitas 70%. Hasil ini menunjukkan adanya korelasi yang signifikan antara rasio neutrofil limfosit (RNL) dengan fraksi ejeksi ventrikel kiri. Simpulan: Terdapat korelasi yang signifikan antara rasio neutrofil limfosit (RNL) dengan fraksi ejeksi ventrikel kiri pada pasien gagal jantung di RSUD Dr. H. Abdul Moeloek Provinsi Lampung tahun 2023. The neutrophil lymphocyte ratio (NLR) is one of the easy and rapid markers of inflammation in various conditions, especially cardiovascular disease. The purpose of this study was to determine the relationship between the neutrophil lymphocyte ratio (NLR) and left ventricular ejection fraction in heart failure patients. Method: This study is an analytical observational study with a cross-sectional design. Data were taken using a purposive sampling technique. The data used were secondary data of 88 medical records that qualified the inclusion and exclusion criteria. This study used the Spearman Rank analysis test. Medical record data were taken at the medical record installation of Dr. H. Abdul Moeloek Hospital, Lampung Province throughout 2023. Result: A sig. (2-tailed) of 0.006 (<0.05) was obtained between the neutrophil lymphocyte ratio and left ventricular ejection fraction with a correlation coefficient of -0.290. Based on the ROC analysis, the best NLR cut-off point as a predictor of left ventricular ejection fraction was 2.67 with a sensitivity of 70.7% and a specificity of 70%. These results indicate a significant relationship between the neutrophil-lymphocyte ratio (NLR) and left ventricular ejection fraction. Conclusion: There is a significant relationship between the neutrophil lymphocyte ratio (NLR) and left ventricular ejection fraction in heart failure patients at Dr. H.
