Insiden jatuh pada pasien menjadi tantangan yang signifikan bagi institusi kesehatan di seluruh dunia, disebabkan oleh tingginya frekuensi kejadian, risiko yang terkait, serta beban biaya yang harus ditanggung. Penelitian ini bertujuan untuk mengevaluasi pelaksanaan pencegahan terhadap pasien berisiko jatuh yang dilakukan oleh perawat di instalasi rawat inap. Metode yang digunakan dalam penelitian ini adalah pendekatan kualitatif dengan desain studi kasus. Dua puluh informan terdiri dari 10 perawat pelaksana, 5 kepala ruang, 1 kepala bidang keperawatan, 1 sekretaris peningkatan mutu dan keselamatan pasien (PMKP), serta 3 pasien yang sedang dirawat direkrut secara purposive. Penelitian dilaksanakan mulai Februari s.d 25 September 2024 di instalasi rawat inap RSUD dr. H. Bob Bazar, SKM di Kabupaten Lampung Selatan. Analisis data menggunakan metode Framework Analysis. Hasil penelitian menunjukkan bahwa pelaksanaan pencegahan terhadap pasien berisiko jatuh oleh perawat di Instalasi Rawat Inap RSUD dr. H. Bob Bazar, SKM Kabupaten Lampung Selatan telah dilakukan dengan cukup baik, yang terlihat dari penyediaan fasilitas, kelengkapan pengisian formulir pengkajian pasien jatuh, serta adanya standar operasional prosedur dan kebijakan yang relevan. Namun, masih terdapat beberapa kelemahan, seperti 26 tempat tidur yang tidak aman karena tidak dilengkapi dengan pagar pengaman, tiga brankar yang tidak memenuhi standar keamanan di UGD, serta kurangnya alas kaki untuk pasien di kelas 3. Selain itu, gelang penanda risiko jatuh berwarna kuning di UGD sering kali tidak tersedia, dan belum semua perawat mendapatkan pelatihan mengenai penerapan pencegahan untuk pasien berisiko jatuh. The incidence of falls in patients is a significant challenge for healthcare institutions worldwide due to the high frequency of occurrence, associated risks, and the burden of costs that must be borne. This study aims to evaluate the implementation of prevention measures for patients at risk of falling carried out by nurses in inpatient installations. The method used in this study was a qualitative approach with a case study design. Twenty informants consisting of 10 implementing nurses, five heads of rooms, one head of nursing, one secretary of quality improvement and patient safety (PMKP), and three patients being treated were recruited purposively. The study was conducted from February to August 2024 at the inpatient installation of Dr. H. Bob Bazar Hospital, SKM, in South Lampung Regency. Data analysis used the Framework Analysis method. The results of the study showed that the implementation of prevention of patients at risk of falling by nurses at the Inpatient Installation of Dr. H. Bob Bazar Hospital, SKM, South Lampung Regency, has been carried out quite well, as seen from the provision of facilities, completeness of filling out the patient fall assessment form, and the existence of relevant standard operating procedures and policies. However, there are some weaknesses, such as 26 unsafe beds because they are not equipped with safety rails, three stretchers that do not meet safety standards in the ER, and a lack of footwear for class 3 patients. In addition, yellow fall risk bracelets in the ER are often unavailable, and not all nurses have received training on implementing prevention for patients at risk of falling.
