Background: Abnormal uterine bleeding (AUB) is a common clinical symptom in gynecological practice. Endometrial histopathological examination is required to detect endometrial abnormalities, including premalignant lesions such as atypical hyperplasia/endometrial intraepithelial neoplasia (EIN). This study aims to analyze the relationship between age and body mass index (BMI) with endometrial histopathological findings in AUB patients. Methods: This is an observational analytical study with a cross-sectional design using secondary data from medical records of AUB patients who underwent endometrial histopathological examination at RSUD Dr. H. Abdul Moeloek Lampung Province, during the period 2021–2025. The sampling technique was total sampling with a sample size of 35 patients. The analysis of relationships was performed using Fisher's exact test. Results: The majority of patients were aged ≥45 years (62.9%) and had BMI <30 kg/m² (74.3%). The most common histopathological finding was non-atypical hyperplasia (77.1%), while atypical hyperplasia/EIN was observed in 22.9%. Fisher's exact test showed a significant relationship between age (p = 0.015; OR = 1.57; 95% CI: 1.146–2.155) and BMI (p = 0.015; OR = 9.58; 95% CI: 1.613– 56.952) with endometrial histopathological findings. Conclusion: There is a significant relationship between age and BMI with endometrial histopathological findings in AUB patients. Latar Belakang: Perdarahan uterus abnormal (PUA) merupakan keluhan klinis yang sering ditemukan di praktik ginekologi. Pemeriksaan histopatologi endometrium diperlukan untuk mendeteksi kelainan endometrium, termasuk lesi pramaligna seperti hiperplasia atipik/endometrial intraepithelial neoplasia (EIN). Penelitian ini bertujuan menganalisis hubungan usia dan indeks massa tubuh (IMT) terhadap gambaran histopatologi endometrium pada pasien PUA. Metode: Penelitian analitik observasional dengan desain cross-sectional menggunakan data sekunder rekam medis pasien PUA yang menjalani pemeriksaan histopatologi endometrium di RSUD Dr. H. Abdul Moeloek Provinsi Lampung periode 2021–2025. Teknik pengambilan sampel adalah total sampling dengan jumlah sampel 35 pasien. Analisis hubungan menggunakan uji Fisher exact. Hasil: Mayoritas pasien berusia ≥45 tahun (62,9%) dan memiliki IMT <30 kg/m² (74,3%). Gambaran histopatologi terbanyak adalah hiperplasia non-atipik (77,1%), sedangkan hiperplasia atipik/EIN sebesar 22,9%. Uji Fisher exact menunjukkan hubungan bermakna antara usia (p=0,015; OR=1,57; IK95%: 1,146–2,155) dan IMT (p=0,015; OR=9,58; IK95%: 1,613–56,952) dengan gambaran histopatologi endometrium. Kesimpulan: Terdapat hubungan yang signifikan antara usia dan IMT terhadap gambaran histopatologi endometrium pada pasien PUA.
