Abstract
Background: Rheumatoid arthritis is a chronic autoimmune inflammatory disease that requires long-term therapy to control inflammation and prevent joint damage. Appropriate therapy based on clinical guidelines is expected to support the achievement of disease activity control targets in patients with rheumatoid arthritis. This study aimed to determine the association between therapy appropriateness and the level of disease activity in rheumatoid arthritis based on the Clinical Disease Activity Index (CDAI) score. Methods: This study employed a retrospective observational descriptive-analytic design with a cross-sectional approach involving 30 patients with rheumatoid arthritis at the Outpatient Clinic of RSUD Dr. H Abdul Moeloek Bandar Lampung. Bivariate analysis was performed using Fisher’s Exact Test. Results: The result showed that 11 patients (36.7%) received therapy appropriate according to the Indonesian Rheumatology Association (IRA) guidelines, while 19 patients (63.3%) received therapy that was not appropriate. In the appropriate therapy group, most patients had low disease activity (23.3%), whereas in the inappropriate therapy group, a larger proportion of patients also had low disease activity (46.7%). Bivariate analysis showed that there was no significant association between therapy appropriateness and disease activity level based on the CDAI score (p = 0.687). Conclusion: The appropriateness of therapy based on IRA guidelines is not directly associated with the level of disease activity in rheumatoid arthritis. Keywords: CDAI Score, Disease Activity Level, Rheumatoid Arthritis, Therapy Appropriateness Latar belakang: Artritis reumatoid adalah kondisi inflamasi autoimun kronis yang memerlukan terapi jangka panjang untuk mengendalikan inflamasi dan mencegah kerusakan sendi. Kesesuaian terapi dengan pedoman diharapkan dapat mendukung pencapaian target pengendalian aktivitas penyakit artritis reumatoid. Tujuan penelitian ini yaitu untuk melihat hubungan antara kesesuaian terapi dengan tingkat aktivitas penyakit artritis reumatoid berdasarkan skor CDAI (Clinical Disease Activity Index). Metode: Penelitian ini menggunakan desain deskriptif-analitik secara retrospektif observasional dengan pendekatan cross-sectional pada 30 pasien artritis reumatoid di Instalasi Rawat Jalan RSUD Dr. H Abdul Moeloek Bandar Lampung. Analisis hubungan bivariat menggunakan uji Fisher’s Exact Test. Hasil: Hasil penelitian menunjukkan bahwa 11 pasien (36,7%) menerima terapi yang sesuai pedoman IRA (Indonesian Rheumatology Association) dan 19 pasien (63,3%) tidak sesuai. Pada kelompok terapi sesuai, sebagian besar pasien memiliki aktivitas penyakit rendah (23,3%), sedangkan pada kelompok terapi tidak sesuai juga ditemukan proporsi aktivitas penyakit rendah yang lebih besar (46,7%). Analisis bivariat menunjukkan tidak terdapat hubungan antara kesesuaian terapi dengan tingkat aktivitas penyakit berdasarkan skor CDAI (p=0,687). Simpulan: Kesesuaian terapi berdasarkan pedoman IRA tidak secara langsung berhubungan dengan tingkat aktivitas penyakit artritis reumatoid.
