N2  - Aplikasi pembuatan kuesioner untuk survei berbasis web adalah aplikasi yang dapat digunakan untuk memudahkan penyelenggara survei (surveyor) dalam pembuatan kuesioner, pendistribusian kuesioner dan perhitungan hasil survei. Aplikasi ini memberikan efektifitas dan efisiensi dalam hal biaya, waktu dan tenaga karena penyelenggara survei dapat mendistribusikan kuesioner secara online dan tidak perlu melakukan perhitungan secara manual untuk mendapatkan hasil survei. Hasil survei yang ditampilkan pada aplikasi ini berupa grafik batang horizontal yang merepresentasikan jumlah responden yang memilih suatu pilihan jawaban pada setiap pertanyaan. Aplikasi ini dibangun menggunakan bahasa pemrograman PHP dan MySQL sebagai media penyimpanan data (database). Hasil pengujian fungsional menunjukkan bahwa fungsi-fungsi pada aplikasi berjalan dengan baik dan sesuai dengan kebutuhan. Hasil pengujian non fungsional menunjukkan bahwa aplikasi termasuk ke dalam kategori ?Baik? dengan nilai persentase yang diperoleh adalah 80,5%.

abstract

Application of questionnaire making for web-based survey is an application that can be used to facilitate survey organizers (surveyors) in procuring surveys, distributing questionnaires and calculating survey results. This application provides the effectiveness and efficiency in terms of cost, time and effort because the survey organizers can distribute the questionnaires online and do not need to do the calculations manually to obtain the survey results. The survey results that shown by the application are in the form of horizontal bar graph that represents the number of respondents who choose the answer for each question. This application was built by using PHP programming language and MySQL as data storage media (database). Functional testing result indicates that the functions of the application are running properly and corresponding with the requirements. Non-functional testing result indicates that the application is included in category of "Good" with obtained percentage value is 80,5%.
