The stages of development were research and information collecting, student’s worksheet prepartion, student’s worksheet validation, preliminary field testing and main field testing.The research subject was grade XI students in SMAN 1 Purbolinggo in academic year 2016/2017.The data of this research were obtained by observation, interview, mathematical creative thinking ability test and self concept scale. The student’s worksheet development was valid according to material and media expert. The result of main field testing showed that the mathematical creative thinking ability and self concept of students that used student’s worksheet based on inquiry more than mathematical creative thinking ability and self concept of students that didn’t use student’s worksheet based on inquiry. In conclusion, the student’s worksheet based on inquiry was effective to increase mathematical creative thinking ability and self concept of students. The improvement of students' mathematical creative thinking ability using LKPD based on inquiry including high category while the improvement of student’s self concept using LKPD based on inquiry including medium category. Keywords: creative thingking, student’s worksheet, inquiry, self concept ABSTRAK Penelitian ini bertujuan untuk mengembangkan LKPD berbasis inkuiri dan menguji efektivitasnya terhadap kemampuan berpikir kreatif matematis dan self concept siswa. Tahapan pengembangan ini dimulai dari studi pendahuluan, penyusunan LKPD, validasi LKPD, uji coba lapangan awal, dan uji lapangan. Subjek penelitian ini adalah siswa kelas XI SMA Negeri 1 Purbolinggo Tahun Pelajaran 2016/2017. Data penelitian diperoleh melalui observasi, wawancara, tes berpikir kreatif matematis dan skala self concept. LKPD berbasis inkuiri yang dikembangkan telah valid menurut ahli materi dan ahli media. Hasil uji lapangan menunjukkan bahwa kemampuan berpikir kreatif matematis dan self concept siswa yang menggunakan LKPD berbasis inkuiri lebih tinggi daripada kemampuan berpikir kreatif dan self concept siswa yang tidak menggunakan LKPD berbasis inkuiri, sehingga dapat disimpulkan bahwa LKPD berbasis inkuiri efektif untuk meningkatkan kemampuan berpikir kreatif matematis dan self concept siswa. Peningkatan kemampuan berpikir kreatif matematis siswa yang menggunakan LKPD berbasis inkuiri dikategorikan tinggi sedangkan peningkatan self concept siswa yang menggunakan LKPD berbasis inkuiri dikategorikan sedang.
