Hemoglobin undergoes destruction due to abnormalities in the synthesis of hemoglobin or globin chains. This study aims to explore the correlation between hemoglobin A2 (HbA2) levels and erythrocyte indices in thalassemia patients at Dr. H. Abdul Moeloek Regional General Hospital, Lampung Province, in 2023. Methods: This research employed an analytical observational study with a crosssectional approach using a total sampling technique. The sample consisted of 39 beta-thalassemia patients who met the inclusion and exclusion criteria. The study was conducted from September to November 2024 in the Medical Records Department of Dr. H. Abdul Moeloek Regional General Hospital, Lampung Province. Data were analyzed using univariate and bivariate methods, with Spearman's correlation test used to determine the relationship between HbA2 levels and each erythrocyte index. Results: The majority of subjects showed elevated HbA2 levels (66.7\%) and decreased MCV (97.4\%) and MCH (92.3\%) levels. A significant correlation was found between HbA2 levels and MCV (p = 0.001; r = -0.525) as well as between HbA2 levels and MCH (p = 0.001; r = -0.508). Conclusion: This study demonstrates that elevated HbA2 levels correlate with decreased MCV and MCH values in beta-thalassemia patients, while no significant correlation was observed with MCHC.
