Medication errors in patients with type 2  diabetes mellitus are high due to the length and complexity of treatment. The  purpose of this study was to determine the medication errors that occurred in the  Pharmaceutical Installation of the Puskesmas Kedaton Bandar Lampung in the  prescribing phase in patients with type 2 diabetes mellitus and the relationship  between the number of therapies with medication errors.  Methods: Observational analytic research with cross sectional design. Sampling    was carried out using purposive sampling method by taking recipes from July-  September 2023 at the Puskesmas Kedaton Pharmacy Installation. Data analysis    using the Chi-Square test and presented in tabular form.  Results: The incidence of medication error was 77.5% which occurred in the  inscriptio, prescriptio, subscriptio and pro sections. There is a relationship between  the amount of therapy and the incidence of medication errors at the Puskesmas  Kedaton with a p-value of 0.046 (0.046 <0.05) and obtained a POR value = 2.743  which means that patients who get the amount of therapy> 4 drugs have a 2 times  higher chance of experiencing medication errors than patients who receive therapy  < 5 drugs.  Conclusion: There is a significant relationship between the number of therapies  and the incidence of medication errors. ABSTRAK    Latar Belakang : Medication error merupakan kegagalan yang tidak disengaja  dalam proses pengobatan yang merugikan atau membahayakan pasien. Medication  error pada pasien diabetes melitus tipe 2 tergolong tinggi karena lama dan  kopleksnya pengobatan. Tujuan penelitian ini adalah untuk mengetahui medication  error yang terjadi di Instalasi Farmasi Puskesmas Kedaton Bandar Lampung pada  fase prescribing pada pasien diabetes melitus tipe 2 dan hubungan jumlah terapi  dengan medication error.  Metode : Penelitian analitik observasional dengan rancangan cross sectional.  Pengambilan sampel dilakukan menggunakan metode purposive sampling dengan  mengambil resep bulan Juli-September 2023 di Instalasi Farmasi Puskesmas  Kedaton. Analisis data menggunakan uji Chi-Square dan disajikan dalam bentuk  tabel.  Hasil : Angka kejadian medication error sebesar 77,5% yang terjadi pada bagian  inscriptio, prescriptio,subscriptio dan pro. Terdapat hubungan antara jumlah terapi  dengan kejadian medication error di Puskesmas Kedaton dengan nilai p-value  0,046 (0,046< 0,05) dan didapatkan nilai POR=2,743 yang berarti pasien yang  mendapatkan jumlah terapi > 4 obat mempunyai peluang 2 kali lebih tinggi  mengalami medication error dibandingkan pasien yang menerima terapi < 5 obat.  Kesimpulan : Ada hubungan yang bermakna antara jumlah terapi dengan kejadian  medication error.  Kata Kunci : Diabetes melitus tipe 2, jumlah terapi, medication error, prescribing
