N2  - Background: Chronic obstructive pulmonary disease (COPD) is a problem in the
lungs due to damage to the airway or alveolus. Exacerbations may also affect the
patient's clinical condition. The economic burden of COPD is huge, especially
hospital costs. In fact, the Indonesian case-based groups (INA-CBG's) tariff is often
not in accordance with real costs. The aim of the study was to determine the cost of
illness in COPD patients.
Methods: The type of research used was retrospective observational with a crosssectional
approach.
The
sample
in
this
study
were
95 COPD
patients
who
received

inpatient
services
at
the
Regional
General
Hospital
Dr.
H.
Abdul
Moeloek
Lampung

Province

in January-March 2025. Data collection technique with purposive
sampling.
Results: The results of secondary data processing obtained the total real costs
incurred by COPD patients amounted to IDR642.811.220. The difference between
the total INA-CBG's tariff and real costs is -IDR62,406,220. The cost of patients
with severe exacerbations was IDR6,172,513 or almost equivalent to 2 times the
cost of patients with mild exacerbations.
Conclusion: The average cost of illness incurred by COPD patients at the Regional
General Hospital Dr. H. Abdul Moeloek Lampung Province was IDR6.766.434
each episode of care.

Latar Belakang: Penyakit paru obstruktif kronik (PPOK) adalah permasalahan
pada paru-paru akibat kerusakan saluran napas atau alveolus. Adanya eksaserbasi
juga dapat memengaruhi kondisi klinis pasien. Beban ekonomi PPOK sangat besar
terutama biaya rumah sakit. Kenyataanya tarif indonesian case based groups (INACBG's)

seringkali tidak sesuai dengan biaya riil . Tujuan penelitian untuk
mengetahui jumlah cost of illness pada pasien PPOK.
Metode: Jenis penelitian yang digunakan retrospektif observasional dengan
pendekatan cross-sectional. Sampel dalam penelitian ini sebanyak 95 pasien PPOK
yang menerima layanan rawat inap di Rumah Sakit Umum Daerah Dr. H. Abdul
Moeloek Provinsi Lampung pada Januari-Maret 2025. Teknik pengambilan data
dengan purposive sampling.
Hasil: Hasil pengolahan data sekunder didapatkan total biaya riil yang dikeluarkan
pasien PPOK sebesar Rp642.811.220. Selisih total tarif INA-CBG's dan biaya riil
adalah -Rp62.406.220. Biaya pasien dengan eksaserbasi berat sebesar Rp6.172.513
atau hampir setara 2 kali biaya pasien eksaserbasi ringan.
Kesimpulan: Rata-rata cost of illness yang dikeluarkan pasien PPOK di Rumah
Sakit Umum Daerah Dr. H. Abdul Moeloek Provinsi Lampung sebesar
Rp6.766.434 per episode perawatan.
