RSIA Belleza Kedaton requires information technology to carry out all activities as supporting health services. Information technology service audit is needed to determine the extent of information technology services at the RSIA Belleza Kedaton. This study uses the COBIT 5 framework to determine the extent of information technology capability levels at the RSIA Belleza Kedaton. Data from this study were obtained based on the results of interviews, observations, and questionnaires. The results show that the capability level of the findings in the company is 2.7 which means it is at level 3 (Established). Thus, the applied process conditions already have a standard and are able to achieve the results of the processes implemented. The results of the capability level of the whole process have not reached the desired level. This difference occurs because several processes in the IT field have not been carried out maximally, there is no scheduling, review, and report on the results of problem solving. Keywords: IT sevice of audit, COBIT 5, Capability Level. RSIA Belleza Kedaton membutuhkan teknologi informasi untuk menjalankan seluruh kegiatan sebagai penunjang pelayanan kesehatan. Audit layanan teknologi informasi dilakukan untuk mengetahui sejauh mana layanan teknologi informasi di RSIA Belleza Kedaton. Penelitian ini menggunakan framework COBIT 5 untuk mengetahui sejauh mana capability level teknologi informasi di RSIA Belleza Kedaton. Data dari penelitian ini didapatkan berdasarkan hasil wawancara, observasi dan kuisioner. Hasil pengukuran menunjukkan bahwa capability level hasil temuan pada perusahaan bernilai 3,0 yang artinya berada pada level 3 (Established). Dengan begitu, kondisi proses yang diterapkan sudah memiliki standar proses dan mampu mencapai hasil dari proses yang diimplementasikan. Hasil capability level keseluruhan proses belum mencapai expected level. Perbedaan tersebut terjadi karena beberapa proses pada bidang TI belum dilakukannya secara maksimal, belum adanya penjadwalan, review, dan laporan dari hasil penyelesaian masalah.
